When I relax my hand next to the keyboard, the most natural position is with the outside of my pinky against the table and my hand sideways, index finger and thumb facing up instead of parallel to the table. When my hand holds a horizontal mouse it often hurts after a short time, with the pain going up the outside of my forearm from my pinky finger, burning and cramping.

It's hard to understand why nobody designed a mouse in the "hand shaking" position, because it's way way more comfortable in the short and long run. Even if this has been around for a while, I've never seen anybody use it.
